Home Care Program
For individuals who are 60 years of age and older, the home care program offers a broad range of services designed to keep them living at home safely and independently for as long as possible. Greater Springfield Senior Services' professional Case Managers and Registered Nurses provide the link to these services, which may be offered by GSSSI or other agencies it contracts with. Eligibility is determined by age, income and functional impairment.
Through a comprehensive assessment, Case Managers determine what services are needed, develop individualized service plans, arrange for the services, and monitor them to ensure clients receive the care they need. Service is available to eligible elders either free of charge or on a cost-shared basis according to income.
