Care Alliance of Western Mass (CAWM)

The CAWM program provides Long-Term Services and Supports Community Partners (LTSS CP) to oversee wellness and independence of individuals through services and resources available in community. LTSS CP work alongside primary care doctors to secure and deliver specialized services required for independent living.

Eligibility:

  • Applicants must have MassHealth Standard or Common Health
  • Applicant’s PCP must be affiliated with an Accountable Care Organization program (ACO) or Managed Care Organization program (MCO)
  • May range between the ages of 3 and 64 years old

Services:

  • Assistance with obtaining community resources and services
  • Assistance with establishing individualized goals and guidance to meet the goals.
  • Provide social support and guidance
