One Care

One Care is an all-in-one health plan for adults with disabilities ages 21-64 years who have MassHealth and Medicare.  Participants (known as members), may choose to work with a Long Term Service Coordinator (LTSC). The LTSC helps the member access services and resources designed to keep the member safe, healthy and independent.

The LTSC is not an employee of the One Care plan, but is a part of the Care Team and will advocate for the members' needs and preferences.

Who is Eligible?

  • Must be between the ages of 21 and 64
  • Must have Medicare Parts A and B, and qualify for Part D (drug coverage)
  • Must have MassHealth Standard or MassHealth CommonHealth
  • Must not have any private health insurance, or be a participant of  a Home and Community-Based Services (HCBS) waiver

People who are dually eligible for MassHealth and Medicare who would like to enroll in a One Care plan should call MassHealth customer service at 800-841-2900.

Existing One Care members who are interested in having an LTSC at GSSSI should contact their Care Partner or Care Coordinator from the One Care plan to make a request. A referral will be made directly to GSSSI and an LTSC will contact the member within 72 hours.
