Request for Proposals for Home Care Program Services

Greater Springfield Senior Services, Inc. is conducting an open bid for the purchase of the following services for its Home Care Programs: Homemaker, Personal Care, Supportive Home Care Aide, Home Health Services (HHA, Skilled Nursing, OT, PT), Chore, Companion, Supportive Day Programs, Adult Day Health, Habilitation Therapy (Alzheimer’s Coaching), Laundry, Grocery Shopping and Delivery, Transportation, Environmental Accessibility Adaptations, Vision Rehabilitation, Emergency Shelter, Short Term Respite Care, Wanderer Locator Service, Personal Emergency Response System (PERS), Enhanced PERS, Cellular PERS, Medication Dispensing System, Home Delivery of Pre-Packaged Medication, Translation/Interpreting Services, Goal Engagement, Orientation & Mobility, and Peer Support Services.

Geographic area served by GSSSI includes: Agawam, Brimfield, East Longmeadow, Hampden, Holland, Longmeadow, Monson, Palmer, Springfield, Wales, West Springfield, and Wilbraham.

Request for Proposal instructions will be available through the FY 2022 RFP link at www.gsssi.org.  Completed proposals are due no later than 12:00 Noon, Friday, June 24th, 2022. Contracts will be awarded to successful applicants from January 1st, 2023 to December 31st 2025, with subsequent renewals possible.

 

Greater Springfield Senior Services, Inc. reserves the right to reject any or all proposals or call for new proposals.
