Community Transition Liaison Program

The Community Transition Liaison Program (CLTP) is available to all nursing home residents who are 22 years old and older, regardless of length of stay or insurance type, and who are interested in returning to community living.

The CTLP Team will work with residents to understand their goals for returning to the community and help with discharge planning by connecting residents to state programs and local community supports. CTLP staff can also assist the resident in advocating and working to resolve concerns related to transitioning to the community.
