Home Care

The State Home Care Program provides services to eligible older residents of our twelve town service area who need assistance to continue to live independently and safely at home. Reduced fees for service are based on income, as well as other mitigating factors and are mandated by state regulation.

Case Managers and Registered Nurses provide free in-home-assessment, care planning, information about resources, and ongoing coordination and monitoring of services. GSSSI contracts with a network of providers to perform the agreed upon services.

Who is Eligible?

Anyone 60 years of age and older is eligible for a free in-home assessment and personalized package of information.

Consumer Direct Care (CDC)

Consumer Direct Care is a program available to eligible participants interested in directly hiring their support care personnel. Once an eligible participant has selected to participate in the CDC program, that individual becomes the employer and can be responsible for recruiting, hiring, scheduling and training their direct care worker.

Enhanced Community Options (ECOP) and Community Choices (CHOICES) Programs

These programs are for individuals who become clinically eligible for a nursing home and need a higher level of services to stay in their home. It is available to MassHealth and Non-MassHealth consumers.

Alzheimer's and Dementia Related Diseases (ADRD) Care Management

This is a program for people with complex and high risk Alzheimer's and Dementia Related Diseases and their caregivers. Our Certified Dementia Care Practitioner recommends, coordinates and monitors contracted services and community supports to promote independence and prevent institutionalization.

Advocacy & Navigating Care in the Home with Ongoing Risks (ANCHOR)

This program provides a higher level of care management that offers more rigorous and time intensive support. It is for people with behavioral health needs that put themselves at risk due to their inability to accept or retain services.
